Include supporting documentation
Attach patient demographics, insurance information, and relevant chart notes supporting the diagnosis, medical necessity, and expected duration of need.

Submit your prescription
Submit the completed prescription form and supporting documentation to your patient's preferred medical equipment supplier. You may also submit the prescription form directly to Liberator Medical SupplyTM, a wholly owned subsidiary of BD®, by fax at 1-800-372-6680.

- Contact the patient
- Verify insurance coverage and benefits
- Review documentation requirements
- Request additional information if needed

What happens next?
Once your medical equipment supplier receives your prescription, their team will review the documentation, contact the patient, verify insurance eligibility, and coordinate fulfillment. If additional information is needed, they will reach out to the prescribing office.
Medicare reimbursement may be available
Eligible patients may be eligible to receive Medicare reimbursement for the PureWick® Urine Collection System and recurring accessories.
